Common causes
- A bug or crash in the PBX or SBC
- A configuration error in the dialplan or routing
- A database or dependency the server relies on is down
- Resource exhaustion such as memory or file handles
How to fix it
- Check the server logs at the far end for the real error
- Fix the configuration or roll back a recent change
- Confirm databases and dependencies are healthy
- Restart the affected service if it is stuck
