Common causes
- The dialed number or extension is not provisioned
- The Request-URI host is the wrong domain
- The dialplan or routing table has no match for the number
- A DID is pointed at the wrong destination
- A simple typo in the number
How to fix it
- Confirm the number or extension actually exists on the target system
- Check the domain in the Request-URI is correct
- Fix the dialplan or routing so the number matches
- Verify with the carrier that the DID is routed to you
